ALL ATHLETES MUST HAVE AN ATHLETIC PHYSICAL ON FILE IN THE ATHLETIC TRAINER'S OFFICE IN ORDER TO TRYOUT FOR ANY SPORT OR PARTICIPATE IN OPEN GYM.  The physical must be dated on or after May 1, 2011 in order for the athlete to tryout or participate in sports at anytime during the upcoming school year.  Students can pick up the physical form from Mr. Sennett (athletic trainer), Ms. Mattson (athletic director) or in the main office.  The form is also available on this Home Page - please see link to the left to download the physical form.
